Accessing Clinical Records

 

Is there a cost involved?
There is no cost for accessing your personal health information.

 

Information collected by Lakes District Health Board from or about you will assist staff in providing you with the best possible care.

You may have access to and request correction of any personal information held by Lakes District Health Board.


Our Location and Opening Hours

Rotorua Hospital
Located in Bridgman South on the ground floor next to the staff entrance, overlooking Kuirau Park.   The department is open from 7.30am to 5.00pm Monday to Friday (excluding public holidays).

Taupo Hospital
The receptionist at the main entrance will direct you. The department is open from 8.00am to 4.30pm Monday to Friday (excluding public holidays).

Please note that requests for information held by Mental Health and Addictions Services in Rotorua and Taupo should be made directly to those services. 

How long do we keep clinical records?
We endeavour to retain all patient records relating to identifiable individuals for a period of twenty (20) years from the last date of patient contact with Lakes DHB.   If you have not had treatment at our hospital for 20 years or more your records may have been destroyed.

How do I get access to my information?
If you are in hospital and wish to view your health information, talk to your nurse or doctor.

If you are not in hospital and require information, write to the Clinical Records Department at Rotorua or Taupo Hospital with your request.

You can also visit the Clinical Records Department during business hours as mentioned above.  You will need to provide some form of identification e.g. drivers licence, community services card or the pages of your passport showing your name and signature.

What about records of people who have died?
If you are requesting information about a person who is deceased it is Lakes District Health Board's policy to only release this information with the written consent of the executor or administrator of the deceased's estate.  The records of patients known to be deceased are retained for 10 years from the date of death.

How long will it take?
Lakes District Health Board is required to respond to your request within 20 working days but will endeavour to respond to your request sooner if possible.
