Pokapū o te Taiwhenua Network
If your clinician has offered you video appointment and you would like that video appointment but do not have the ability to connect, with your consent, your clinician can refer you to Pokapū o te Taiwhenua Network.
The Pokapū o te Taiwhenua Network is a virtual network of health and wellbeing providers seeking to support your access to your video appointment in your home or local community no matter who you are or where you live in the Te Whatu Ora Lakes area.
As soon as a referral is received, our Pokapū o te Taiwhenua Coordinator will make contact with you to start this process.
Additionally, the Pokapū o te Taiwhenua Network can support digital health coaching so you know what options are available to you.
