Early learning service gastro or respiratory outbreak notifications — Greater Wellington

Complete this form when you suspect you have an outbreak at your early learning service in Greater Wellington.


An outbreak is when either of the following are true:

  • 3 or more cases, including staff and tamariki, in one area have similar symptoms
  • 15% of the early learning service, including staff and tamariki, are unwell.

Include the name and contact number of the person notifying the outbreak — do not include any other personal health information.

Where there is a serious injury or illness or incident involving tamariki while at the service that is required to be notified to a specified agency, the service provider must also notify the Ministry of Education at the same time.
