Bee Healthy only provides dental care to children up to the end of intermediate school (Year 8).

If your child is Year 9 and above, please contact and enrol them with a dentist that provides free dental care for teenagers.

Eligibility

Proof of eligibility

After submitting this form, Bee Healthy will email you to request documents related to your child’s eligibility e.g. birth certificate, passports, certificates, visas, and other documents.

Please reply to the email with all the required documents attached as we are unable to complete your child's enrolment without them.

Consent

This consent will remain valid while your child continues to receive dental care provided by Bee Healthy Regional Dental Service on a mobile clinic or at a dental hub. Consent can be withdrawn at any time by contacting Bee Healthy Regional Dental Service
